How Do I know If I Qualify for Care?
Community Smiles Dental provides dental healthcare to individuals who meet these eligibility guidelines:
Have BadgerCare/Forward Health/Medicaid insurance OR
Have income that falls below 200% of the Federal Poverty Guidelines and who are uninsured.
For patients without dental insurance, a sliding fee scale applies.

What to Bring to Your Appointment
Uninsured patients or guardians are asked to provide two of the most recent pay stubs for all working adults in the household, along with their most recently filed tax return (ideally from the previous year).
If these documents are not available, our team will work with you to identify alternative forms of documentation. For individuals who are not currently employed, this may include unemployment benefit statements, SSI award letters, or similar records.
A credit card or debit card for payment is preferred.
